Thank you for your interest in applying to enrol your child at Mundaring Primary School. Please complete an Application for Enrolment form by downloading the form below or request a copy from the School Office.
The School welcomes students from Kindergarten to Year 6 and, at present, we may have vacancies in some year groups.
Proof of residence is required
When completing the Application for Enrolment Form you will also be required to provide evidence that you reside within the local intake area for Mundaring Primary School. Applications cannot be considered until such time as documented evidence is provided to the school, (for Kindergarten applications we require the application form only until a placement has been confirmed).
If you reside outside our intake area, you are still able to make an application to enrol. However, it is up to the principal to approve the request.
Applications for entry can be received at any time, and dependent on available places, families will be notified if successful as soon as possible.
Procedure for Enrolment
Parents initially complete an Application for Enrolment (see below for relevant years), then when eligibility for enrolment is confirmed, the Formal School Enrolment Form is completed.

Some schools receive more applications than they have places thus applications are then prioritised in accordance with the Department of Education guidelines and procedures.
Once your "Application for enrolment" has been accepted, you will need to provide us with supporting documentation for each child.

Kindergarten Immunisation Requirements
On 22 July 2019, changes to the Public Health Act 2016 (WA) and the School Education Act 1999 (WA), came into effect that will better protect young and vulnerable WA children and the wider community from vaccine-preventable diseases.
Under these new changes, a child’s immunisation status must be ‘up-to-date’ according to their AIR Immunisation History Statement and must be dated within 2 months of the date of application for enrolment to be permitted to enrol into Kindergarten.
